Is this your first time visiting our facility?

Before your appointment, please ensure you have proof of identification and up-to- date insurance cards. You have the option to fill out all necessary paperwork before your appointment via our downloadable forms.

Please arrive 15 minutes early to your scheduled appointment to ensure that your patient registration is complete. Please note that to provide high-quality evaluation and treatment, comfortable loose-fitting clothing such as sweatshirts, jogging pants, or shorts are recommended.

Understanding your insurance

When discussing insurance, it helps to become familiar with what each term means.

  • Insurance provider/carrier, refers to the company that provides you and your family health insurance.
  • Insured/policy holder, is the individual or family that is provided health insurance.
  • Premium, the amount you pay each month to keep and maintain your health insurance.
  • Deductible, refers to the amount you must cover before your insurance provider begins to pay.
  • Co-payment, is a fixed amount you must pay for covered services, this amount determined based on your insurance policy.
  • Co-insurance, is a percentage of a medical bill you pay, with the rest paid by your health insurance provider.
